Important! – The Application is to be emailed to the District Master SK James DeLuccia, 4th Degree Membership Chairman PSD SK Bruce DeMolli and to the District Secretary Michael Leyden by the Faithful Navigator to: jjdkofc15@gmail.com, bdmolli70@gmail.com and mleyden@adeptmobile.com
Here are the detailed instructions for filling out the 4th Degree Membership Application:
1. For 3rd Degree Members applying for 4th Degree please download the application to your computer first, Adobe Reader is the recommended program. You will need to fill out the following in yellow. In blue is the Naturalization question, if by “Birth” you do not check Yes or No, if Naturalization you check either Yes or No. Enter your Member Number, it is located on your membership card, you may ask your Council Financial Secretary for it as well as for the date you completed your first degree. Start with the Member Number field and after each entry you can use the TAB key on your keyboard to advanced to the next field
2. The Proposer, who is a 4th Degree Member that is sponsoring your membership into the 4th Degree, will fill out the following in yellow.
3. The Council Financial Secretary will fill out the following in yellow.
4. The Application will be presented by the Proposer with #1, #2, and #3 completed to the Assembly Faithful Comptroller to be read to the 4th Degree Membership for acceptance. If accepted, then the Faithful Navigator and Faithful Comptroller will fill out the following (below) at the 4th Degree Meeting in yellow.
